Looking For a New Doctor?

Newborns are cared for by the hospitalists while in the hospital. Be mindful to register your baby under your Southwest Children’s Center pediatrician’s name while at the hospital. This ensures all information is transferred to us. A hospitalist physician will see your baby within 24 hours of delivery and will continue to do so daily until discharge. It is important that all our babies, regardless of where they are delivered, have a clinic appointment within 48 hours of discharge. Call to schedule that appointment prior to leaving the hospital.

For New Families Transferring to Us

Meet and Greet is a great way for prospective parents and new patients to ask questions, see how we work, and meet one of our providers prior to choosing Southwest Children's Center. We want to help you make the best decision for your family. These visits are not required to become a patient of Southwest Children's Center.

If you have chosen southwest children's center, please sign a release of records form and provide it to your children's previous medical provider. 

 

Please Bring the Following to Your First Appointment 

• Any paperwork from the hospital, including hearing screen results and immunization records.
• Any available insurance information (often only the parent’s insurance is available at the first visit).
• Plenty of diapers and an extra set of clothes.
• Please complete the New Patient Forms prior to the visit to help make the appointment as smooth as possible.
